As Discharge Coordinator for Bluff Plantation, Larisa Washington helps patients safely and successfully transition out of residential treatment and back into the community.
She coordinates with patients, treatment teams and family members to develop a plan for discharge, and ensure each individual has the support and resources in place to continue in their recovery. Resources include arranging for step-down treatment, ensuring patients with co-occurring disorders receive ongoing psychiatric care, making sure patients have safe and appropriate housing, and connecting individuals with community-based support networks, such as 12-step programs and faith communities. Larisa also leads a weekly transition planning group to provide education and a forum for discussion about the anxieties and challenges that come with leaving residential treatment, and the importance of having a plan for continuing care.
Larisa is active in the recovery community and has a background in administration for medical facilities. She appreciates the opportunity to help people during a time of positive change in their lives, and seeing them transform into a better version of themselves.
