At Bluff Plantation, a Therapist will be assigned to work with you and manage your case. He or she will collaborate closely with each person involved in your care to develop an addiction treatment plan unique to your needs.
Your Therapist will monitor your progress and amend your plan as needed for maximum success. You will have daily interactions with your Therapist, as he or she advocates for you and coaches you through the various stages of treatment at Bluff Plantation.
Continuing Care Coordinator
At Bluff Plantation, our goal is to provide the skills and resources necessary to our clients for long-term sobriety and well-being. Each client's treatment team will work with a Continuing Care Coordinator to develop your discharge plan and identify the resources necessary to assist in your continued recovery.
Your Continuing Care Coordinator will communicate with external therapists, professional agencies, recovery coaches, potential sober housing facilities, and others who will become crucial to your continued sobriety.
At Bluff Plantation, we recognize that you are the key stakeholder in your well-being. Recovering from addiction requires a long- term commitment that only you can control.
The Most Important Person: You
Certainly the most important person in the recovery process is you. As a member of your treatment team, you are encouraged to be present and participate in team meetings that involve your care. Your role is to be as open and honest as you can about your addiction, significant difficult events during your life, and your innermost feelings, fears and struggles.
